Background check disclosure

Home Happenings may obtain a consumer report from a screening vendor for the purpose of evaluating your provider application and continued eligibility. The report may include criminal background information and a national sex offender registry search.

Screening vendor name, address and telephone: [TO BE ADDED BEFORE SCREENING]. Applicable federal and state notices, including the Summary of Your Rights Under the Fair Credit Reporting Act, will be supplied before obtaining a report. Additional disclosures required by law will be supplied separately.

This document is a standalone disclosure, not a liability waiver or the Provider Agreement. Authorization is collected separately. No consumer report will be obtained from this demo. Legal counsel must finalize the disclosure and applicable rights notices before screening begins.
